Senior Tournament Entry Form

Team, Singles, and Doubles  entries cost $25 each per person.  All events entry cost $5 each for scratch and handicap.  Payment in check or cash can be given to Jan Escalante, Linda Goff, Diane Williams, or Tammy Melendrez.
